Late Registration of Death (After 3 months from the time of death)

Late registration of death and issuance of death certificates are done at any Civil Regisration office countrywide. The relatives of the deceased, insurance companies, employers or estate administrators are persons who qualify for this service . Required; 1. Police report/letter from the chief/ health institution report confirming death 2. Sworn affidavit by applicant confirming death 3. I.D card of the deceased incase of an adult 4. D4/A3 (registration of late deaths)duly completed 5.
