NHIF Membership Registration To Register as an employed person, you will be required to have your Original and Photocopy of ID and introduction letter from your employer, your passport photograph, and particulars of your nuclear family members if applicable - Spouse copy of ID, copy of Marriage certificate/affidavit, Birth Notification(for Children 6 months and below) and Birth Certificate (for Children 7 months and above) and their passport photos . With this you will fill and submit NHIF Member application form at any NHIF office or Huduma Centres countrywide at which point you will be issued with an NHIF card.
